Laptop Loaner Service Application Extension Form

Thank you for your interest in continuing to access Contact North’s Laptop Loaner Service to borrow a laptop at no cost.

We will review your application to confirm if you are eligible to continue borrowing equipment.

Before you fill out the application form below, please make sure you have the following information:

1. 1. Your address for shipping the equipment:
      • Street address
      • Apartment number and buzz code (if you live in an apartment)
      • City or town
      • Postal code
      • Phone number
        
        
   2. The name of the education institution or school.
      
      
   3. The name of at least one online course you are registered in (or have applied for) and the online course code from the institution or school.
      
      
   4. Proof of application or registration in your online course to be uploaded in any of the following acceptable file formats: jpeg, png, heic, docx, doc and pdf.

It can take between 15 – 30 minutes to complete the form.

Please also note that if your application is incomplete, we will reach out to you from [email protected] or 1-877-999-9149 to get the missing information. If we don’t have that information within five business days, your application will be cancelled.
